Walking With The Wounded ‘WWTW’ deliver employment, mental health, care coordination and volunteering programmes in collaboration with the NHS to get those who served, and their families, whether mentally, socially or physically wounded, back on their feet and making a positive contribution once more. This role will work in collaboration with the Cumbria Northumberland Tyne and Wear NHS to support veterans in the Mental Health pathway. As our contribution to the project offering, we will be employing a new Care Coordinator.
The successful candidate will ideally be centrally around Northumberland, Tyne and Wear areas.
WWTW’s Care Coordination service involves the assessment of needs for your client, the sourcing of appropriate support and the ongoing navigation of that support. You remain the central point of contact as you support your service user to address their challenges, which may include a lack of secure housing, debt, social isolation, relationship breakdowns, lack of education and training, welfare issues or substance misuse.
