Hospital passports are used to provide important information about a person with a learning disability in the event they are taken into hospital. It allows health staff to understand the needs of the person and the reasonable adjustment required to support their care and treatment, in particular the covid passport allows the person’s covid related symptoms to be communicated.
To help understand the uptake in South Tyneside, service providers are asked to send feedback to the following questions:
- Service name
- The number of people who have been supported to complete a covid hospital passport.
- The number of people with general hospital passports.
