The aim of the programme is to provide safe and effective physical activity opportunities for people who live in Sunderland, are registered with a Sunderland GP and have a long term medical condition that their GP or other healthcare professional believes could be improved through a bespoke exercise programme. Adults 16 years+ who are currently not physically active and have a long term medical condition or are at risk of long term conditions and chronic diseases, that could be improved through a bespoke exercise programme, and are at the right stage of behaviour change. Primary reasons for referral are: • CHD • COPD • Asthma • Heart Failure • Peripheral Arterial Disease • Atrial Fibrillation • Diabetes • Stroke • Anxiety/Stress/Depression • Falls (via the Falls Clinic) • Joint Pain • Back Pain • Cancer For more information on the programme and information on how do access the programme please contact the administration department on 5614566
