Home from Hospital
Funded by Washington Area Committee this project supported people aged 55+, who live in Washington, Tyne and Wear and had recently been discharged from hospital. Those who fit this criteria were given the opportunity to build their confidence, engage in therapeutic activities, get out and about, meet new people and enhance their social networks.
Washington Mind worked with local partners: The Washington Millennium Centre, Springwell Village Community Hall to deliver the project.
Following the completion of this project we have created a leaflet containing information on local organisations in the Sunderland area who can offer support to people who are aged 55+ and have recently left hospital. To see our leaflet, please click here.
