Washington Healthy Lifestyles Project
Are you 50 years plus, live in Washington and want to make positive lifestyle changes? If so, this programme may be able to help you.
The programme can support people who are at risk of, or are currently, suffering from long term medical conditions, including:
• Hypertension • Osteoarthritis
• Hypercholesterolemia • Rheumatoid Arthritis
• Chronic Obstructive Pulmonary Disease (COPD) • Osteoporosis
• Coronary Heart Disease (CHD) & MI • Simple mechanical back and joint pain
• Stroke • Joint replacement
• Asthma • Mental Health (anxiety, stress and depression)
• Cancer • Hypertension
• Overweight/Obesity • Diabetes
The programme offers a number of options to help you to get started and support you in improving your health and wellbeing. They include:
- Mini MOTs /Health Checks
- Physical activity sessions
- Advice on healthy eating and changing your eating habits
- Signposting to other services i.e. Stop Smoking, Alcohol advice
- Motivational support
